Present valid government-issued photo ID to our commissioner, declare that the contents of your forms and exhibits are true, and sign the documents. You must present one valid, unexpired piece of government-issued photo ID. Accepted forms of ID include an Ontario Driver’s License, Passport, Canadian Permanent Resident Card, or Ontario Photo Card.
