Download the required forms and fill them out completely. If you are attaching evidence (called "exhibits" like bank statements or text messages), organize them and label them clearly. Leave the signature fields completely blank.

Present valid government-issued photo ID to our commissioner, declare that the contents of your forms and exhibits are true, and sign the documents. You can meet with our licensed Commissioner securely via video call, electronically sign your document, and file the finalized PDF directly through the Ontario Justice Services Online portal.
Yes, we can commission both the main affidavit and any supporting exhibits (such as emails, receipts, or photographs) during your appointment. If you are doing this online, simply upload your exhibit files along with your main form before your video call.
No. While we ensure your documents are legally sworn and properly executed, Commissioners of Oaths and Notaries Public cannot provide legal advice regarding the contents of your forms or how they will affect your family law case. You must present one valid, unexpired piece of government-issued photo ID. Accepted forms of ID include an Ontario Driver’s License, Passport, Canadian Permanent Resident Card, or Ontario Photo Card.
